360 BAC Service Agreement

1. Service Description

360 BAC provides professional alcohol monitoring services through portable breathalyzer devices with video verification capabilities.

2. Device Responsibility

You agree to use the provided 360 BAC device responsibly and return it in good condition upon service completion. Any damage or loss will result in replacement fees.

3. Testing Requirements

You must complete all scheduled tests as required by your monitoring program. Missed tests or tampering will be reported to relevant authorities.

4. Data Usage

All test results, videos, and related data may be shared with probation officers, courts, employers, or other authorized parties as required.

5. Payment Terms

Monthly service fees are due in advance. Late payments may result in service suspension and additional fees.

6. Privacy

We collect and store personal information in accordance with our Privacy Policy. Test data is securely stored and transmitted.

7. Compliance

You agree to comply with all federal, state, and local laws regarding alcohol monitoring programs.

8. Service Termination

Either party may terminate this agreement with 30 days written notice. Early termination fees may apply.

By accepting, you agree to:

  • • The 360 BAC Service Agreement and Privacy Policy
  • • Share test data with authorized parties (courts, probation officers, etc.)
  • • Accept full responsibility for the device and replacement fees for damage or loss
  • • Complete all scheduled tests as required by your monitoring program
  • • Pay monthly service fees in advance
